Sydney is the base for a Care Partner / Case Manager with an in-home aged care provider. Its clients are older Australians living in their own homes, supported through the Support at Home program, and it wants a compassionate, experienced person to join a close-knit team.
Working directly with clients, you would lead the coordination of their care and look after individual support plans. The flat team structure gives you room to work autonomously while keeping services timely and right. The provider also offers visa sponsorship to the right candidates.

Key Responsibilities
- Build relationships with clients, families, support workers and outside providers, in a respectful way
- Arrange services around what each client wants and needs for their health, and carry out assessments, writing and reviewing care plans
- Make safety, wellbeing and quality of life the basis for every decision
- Lead and supervise Support Workers, protecting service quality and continuity
- Monitor client budgets and keep to legislation, funding guidelines and internal policies
- Keep timely, accurate progress notes, review notes from others involved, and admit new clients through intake assessments and meet and greets
Requirements & Eligibility
- Experience of aged care, home care, community services or a health setting
- Assessment, care planning and case management skills
- Able to work independently and balance competing priorities
- Strong communication, relationship-building and leadership
- Working knowledge of My Aged Care and the Aged Care Quality Standards, along with a driver's licence, own vehicle and willingness to travel across Sydney
- Languages: bilingual or multilingual ability is a strong advantage; a clinical background (nursing or allied health) is beneficial but not required
